Conflict request form (CRF)

DUE by Feb 18

  • All CRFs must be submitted by February 18th, 2026. If your team does not have a submitted CRF, we will assume you have no conflicts.
  • If you submit a CRF and later need to make changes, please submit another form. We will only accept the most recent submission.
  • You MUST HAVE at least 6 days available.

Add Players / Team Payment

Players must be added by Feb 18 / Payment due at time of player addition

  • At least 3 players must be added by February 18th, 2026 for your team to be approved.
  • A payment of $75 is required when each player gets added to the roster.

PLEASE NOTE THAT THE ABOVE STEPS ARE REQUIRED FOR TEAM APPROVAL

TEAM ROSTER FORM

DUE BEFORE FIRST GAME

  • The Team Roster Form must be completed before first game.
  • Please be sure to include the EXACT TEAM NAME.
  • If you have multiple teams with the same exact roster, you may include all team names in the TEAM NAME box seperated by a comma.
  • If you have already submitted a form and you need to add more players, you can send an email to info@thepitplus.com with the team name and player details, or add them prior to kickoff on gameday (just arrive at least 15 mins early). In addition to the team name, please make sure the email includes the following for each player to be added:
    • Child Name
    • Child Date of Birth
    • Parent Name
    • Parent Phone #
    • Parent Email Address
    • Parent Home Address

Game Day Procedures

  • Each child checking in will need to have a valid $25 Membership and a valid waiver to play. These can be taken care of online or at the front desk.
  • Every player will need to visit the front desk to check in before game time. We will need to know:
    • Team Name or Club
    • Child’s Name
    • Time of Game (if more than an hour away)
  • Once checked in, each player will be given a wristband. They will need to wear that wristband during all of their games that day. Players that have not received a wristband will be sent off before the game can start.
